Personal Data Owner Application Form
GENERAL EXPLANATIONS:
In the Law No. 6698 on the Protection of Personal Data (‘KVKK’), the personal data owners (‘Data Owner’), who are defined as ‘relevant person’ in the Law No. 6698 on the Protection of Personal Data (‘KVKK’), are entitled to apply to the Data Controller, namely Private Durupol Oral and Dental Health Polyclinic San. Tic. Ltd. Şti. (shortly Durupol Oral and Dental Polyclinic) has the right to apply. The applications to be made to us regarding these rights must be submitted to us in writing or electronically and by the following methods in accordance with paragraph 1 of Article 13 of the KVKK and the relevant provisions of the Communiqué on the Procedures and Principles of Application to the Data Controller.
APPLICATION METHODS:
In the applications to be made to us within the scope of the Law on the Protection of Personal Data, in accordance with Article 5/2 of the Communiqué on the Procedures and Principles of Application to the Data Controller, which entered into force after being published in the Official Gazette dated 10.03.2018 and numbered 30356, it is obligatory to include the name, surname and signature if the application is in writing, T.R. identification number for citizens of the Republic of Turkey, nationality, passport number or identification number, if any, for foreigners, residential or workplace address for notification, e-mail address, telephone and fax number, and the subject of the request.
